The Characteristics of Elderly Patients with Hemorrhagic Stroke

Abstract
Objective
The number of people over 75 years of age in South Korea is annually increasing. However, there are very few reports on acute hemorrhagic stroke in elderly patients (aged ≥75 years). The objective of this study was to identify the characteristics of intracerebral hemorrhage (ICH) in older patients.
Methods
We identified patients aged ≥18 years who were admitted with spontaneous ICH between January 1, 2007 and September 30, 2012. We retrospectively collected demographic characters, risk factors of vascular disease, and pre-ICH medication history. We analyzed the characteristics (including hematoma volume, location and expansion) of ICH according to the age of patients.
Results
Of 622 ICH patients, 76 (16.7%) were ≥75 years of age. We found that older age (≥75 years) was significantly related to a larger hematoma volume (median hematoma volume 12 cc (5-32) in aged <75 years patients versus median volume 18 cc (12-46) in elderly group, p=0.042) and a greater hematoma expansion (expansion ratio 23.9% in younger patients versus 40.8% in elderly patients, p=0.002). Initial hematoma volume, hyperglycemia, and a history of AF (atrial fibrillation), DM(diabetes mellitus) and CKD (chronic kidney disease), were found to be statistically related to hematoma expansion in patients ≥75 years of age. The most important predictors of ICH expansion in these patients were hyperglycemia (OR, 0.03; 95% CI, 0.01-0.20; p<0.001) and CKD (OR, 0.15; 95% CI, 0.03-0.80; p=0.026).
Conclusion
In our study, older age (≥75 years) was significantly related to a larger hematoma volume and a greater hematoma expansion. Furthermore, hyperglycemia and a history of CKD were important predictors for ICH expansion in these patients.
