Characteristics of Chronic Subdural Hematoma in Elderly Patients Aged 80 or Older

Abstract
Objective
Chronic subdural hematoma (CSDH) is more common in geriatric population. We need to evaluate CSDH in very elderly because the aging of the population is expected to accelerate in future. The purpose of this study is to analyze clinical characteristics and outcomes of elderly aged 80 or older with CSDH.
Methods
Between January 2005 and January 2013, the authors treated 114 surgical patients with CSDH. Patients were divided into two groups: elderly aged 80 or older in group A, patients with less than 80 years in group B. Clinical presentations, precipitating factors, computed tomography (CT) findings, postoperative complications, and outcomes of patients were retrospectively analyzed.
Results
The mean age of group A was 85.8 years and the mean age of group B was 68.7 years. Hemiparesis (38.9 %) as a presenting symptom was most common in group A, and headache (40.1%) was most common in group B. Systemic complications which were not associated with operation were significantly higher in group A (p=0.031). The modified Rankin Scale (mRS) 0-2 at 3 months postoperatively was 69.4% in group A and 88.5% in group B (p=0.013).
Conclusion
Elderly patients aged 80 or older showed worse outcomes mainly due to high risks for hospital-acquired complications. More concentrated and sophiscated care must be given for the very elderly patients after operation.
