Relationship Between Operation Timing and Resolution Rates in the Spontaneous Intracerebral Hematomas |

Abstract |
Objective Stereotactic aspiration is one of the major surgical modalities for treating spontaneous intracerebral hemorrhage (SICH). Among the SICH patients, some hematomas resolved in a few days, whereas others resolved in a few weeks. We set up a hypothesis that the timing of operation making difference the hematoma resolution rates.
Methods This retrospective study included 44 patients who were treated with stereotactic aspiration. We surveyed several hemodynamic factors including PT, aPTT, CBC, pH, the timing of urokinase irrigation, the timing of hemostatic medication, the time interval between symptom onset and operation (TISO). The hematoma resolution rate was checked by CT scan preoperatively and on the 3rd, 7th and 14th postoperative days. The patients were categorized according to its rapidity of hematoma resolution.
Results The mean age of the patients was 65.9 years. Among the aforementioned factors, only TISO was significantly different between the FR, MR and SR groups. The FR group showed a longer TISO than the MR and SR groups. In addition, the MR group showed a longer TISO than the SR group.
Conclusion The results of this study suggest that the difference in the hematoma resolution rate may depend on the timing of operation, regardless of the repeated use of hemolytic agents or other hemodynamic factors including PT, aPTT, CBC, pH and hemostatic medications. |
