The Mutuality between Age and the Prognostic Factors Influencing on the Surgical Outcomes in Elderly Patients with Ruptured Intracranial Aneurysms: A Comparison with the Young Patients

Abstract
Objective
To evaluate the reciprocal relationship between age and the prognostic factors influenced on the surgical outcomes in patients with aneurysmal subarachnoid hemorrhage according to age.
Methods
385 patients surgically treated in our hospital between 1998 and 2005 are retrospectively reviewed. They were divided into elderly (over 65 years old) and young (below 64 years old) groups, and their clinical characteristics and surgical outcomes were analysed. The outcomes are evaluated in each patient by the modified Rankin Scale three months after operation.
Results
The presence of hypertension, the size and location of aneurysms, and the rate of shunt surgery were affected by age. In both group, prognostic factors influenced on the surgical outcomes were admission grade such as Hunt-Hess grade and World Federation of Neurological Surgeons grade, Fisher grade, vasospasm, and ventriculo- stomy. The size of aneurysm and the shunt surgery areinfluenced on the surgical outcomes in young group, but not in elderly one.
Conclusion
Hypertension was more frequently found in elderly group and there was a slight difference in the location of aneurysm between two groups, however, they were not influenced the surgical outcome in both groups. The proportion of large aneurysm was more common in elderly group than young one, but the surgical outcome of large aneurysm was worse in young group than in elderly one. The rate of shunt surgery was higher in elderly group, however, it was only influenced on the surgical outcome in young group.
