The Usefullness of Unilateral Laminectomy in Intradural-extramedullary Spinal Tumors

Abstract
Objective
To compare the clinical outcomes and postoperative kyphosis of the unilateral laminectomy and total laminec- tomy for primary spinal cord tumors. Methods: From January 2005 to December 2010, 45 patients who underwent surgery for primary spinal cord tumor were divided into 28 patients by unilateral laminectomy and 17 patients by total laminectomy. Results: There were no statistical differences in the histologic features, location and size of the tumors between unilateral laminectomy and total laminectomy. After 6 months postoperation, 85.7% of unilateral laminectomy and 82.4% of total laminectomy showed no recurrence. In the unilateral laminectomy, postoperative Cobb’s angle was 10.65±11.28°. After 6 month, 10.25±9.83° in the sagittal plane of the involved segment level. In the total laminectomy, postoperative Cobb’s angle was 10.71±7.14°. After 6 month, 10.92±8.54° in the sagittal plane of the involved segment level. But there was no statistical difference between the two groups. Conclusion: There was no significant difference in tumor recurrence and spinal deformity between unilateral laminectomy and total laminectomy. If the appropriate technique was chosen according to tumor location, size and characteristics of the patients, both of them could be effective treatment methods in patients with primary spinal cord tumors.
