Comparison of Outcome from Conservative Treatment and Vertebral Augmentation Procedures for Osteoporotic Thoracolumbar Vertebral Compression Fractures in Elderly

Abstract
Objective
Although there has been a great increase in number of vertebral augmentation procedures (percutaneous vertebroplasty and kyphoplasty) for osteoporotic vertebral compression fractures in elderly, there are still much concern about indications and timing of procedures. This study was done to help clinicians to decide whether conservative care or interventions including percutaneous vertebroplasty and kyphoplasty would be more suitable for the patients diagnosed with thoracolumbar compression fractures and when to be carried.
Methods
A total of 99 patients with osteoporotic thoracolumbar vertebral compression fractures who were treated with conservative treatment or intervention were enrolled at our institution for one year period. We divided the patients into three groups. Variables such as age, sex, compression ratio, Bone mineral density (BMD), and treatment outcome were compared and analyzed between these three groups.
Results
Medical comorbidities such as diabetes or hypertension did not affect the outcome but as age, compression ratio, and BMD scores are shown to be important variables to affect outcome when pain scores are similar.
Conclusion
Patients who are older and have more severe osteoporosis are likely to be failed with only conservative treatment. Thus, percutaneous vertebroplasty and kyphoplasty seem to be still important in the treatment of these elderly and osteoporosis patients refractory to conservative treatment.
