The Long-term Outcome of Chronic Subdural Hematoma in the Elderly: 2-Year Follow-up

Abstract
Objective
Chronic subdural hematoma is generally known as benign condition in geriatric patients. However, there are little reports of the long-term outcome in elderly patients with chronic subdural hematoma. We investigated the long-term outcome of the elderly patients over 65 years with chronic subdural hematoma (CSDH).
Methods
From January 2011 to December 2012, the 48 elderly patients with CSDH were analyzed retrospectively. We examined 6-month mortality and 2-year mortality using telephone interview. We assessed the long-term outcome of CSDH of the elderly according to functional dependency at discharge (Glasgow outcome scale score ≤3) using Kaplan-Meier survival curve. Cox proportional regression analysis was performed to assess independent predictor for death of CSDH of the elderly.
Results
Among 48 patients, 6-month mortality was 10.4% and 2-year mortality was 20.8%. A half of CSDH patients with functional dependency at discharge died within 6 months after discharge. Age, serum albumin, and functional dependency at discharge showed statistically significant differences between survival and death group (p≤0.05). Functional dependency at discharge was associated with 2-year mortality after CSDH of the elderly (Hazard ratio 19.99, 95% CI: 1.11-360.37, p=0.04).
Conclusion
CSDH seems to be associated with mortality in the long-term follow-up of the elderly patients. Functional dependency at discharge may be independent predictor of the long-term outcome of the elderly with CSDH. In the future, large prospective analysis may need to investigate the long-term outcome of CSDH.
