Atypical Presentation of Cervical Radiculopathy Due to Pancoast

Abstract |
We report an uncommon case of Pancoast tumor presented as atypical cervical radiculopathy. A 64-year-old man
presented with severe right arm and shoulder pain of 2-month history. At a local clinic, cervical magnetic resonance
(MR) imaging showed cervical stenosis at the C5-6, C6-7 level. He was referred to our institute for cervical disc
surgery. Several days before admission, he experienced exacerbated radiating pain and progressive arm weakness.
During preoperative evaluation, a huge apical mass of the right lung field is found in routine plain cervical anteroposterior
(AP) radiograph. Further computed tomography (CT) scans of the chest showed a huge apical tumor encroaching
right C7-T1 and T1-T2 intervertebral foramen. Percutaneous aspiration cytology confirmed squamous cell
carcinoma of the lung. Radionuclide whole body bone scan showed multiple bone metastasis. Systemic chemotherapy
and radiotherapy for bone metastasis were performed. The importance of early diagnosis and pathomechanism
is discussed with a relevant literature. |
