Changes of Spinal Surgery for Geriatric Patient Through Two Decades in Single Institution

Abstract
Objective
The incidence of spinal surgery, including instrumented fusions, is increasing, and progressively involves patients of age 65 and older. Sometimes many elderly patients are crippled by degenerative spine conditions. We tried to review the changes of spinal surgery for geriatric patients through two decades among 1991, 2001, and 2011 in single institution.
Methods
The medical records of 1991, 2001, 2011 were sorted and reviewed by using code of spinal surgical name in our institution. The cases of spinal surgery over than 65 years old were analyzed. Code of surgical name were sorted each spinal surgery and includes simple discectomy and spinal fusion. Sorted surgical name of 1991 was excluded due to our institution was not had code of surgical name.
Results
Total number of spinal surgery of all ages was 425 cases at 1991, 475 cases at 2001, 971 cases at 2011. Proportion of spinal surgery in geriatric patients was 6.3% at 1991, 14.1% at 2001, 40.7% at 2011. Simple discectomy for geriatric patients was reduced (59%), however, spinal fusion was increased to 296%. Decompressive laminectomy and kyphoplasty due to osteoporotic spinal compression fracture were also increased in geriatric patients. Clinical results and complication according to ages were not different.
Conclusion
We reviewed and analyzed changes of spinal surgery for 20 years. Increase of geriatric spinal surgery was distinctly seen. There may be many presumptive reasons for increase including geriatric spinal surgery. This may be help for preparation of the time of 10 years later in spinal surgery patients.
