The Effect of Nerve Block on Spinal Pain in Extremely Elderly Patients Over 75 Years of Age |

Abstract |
Objective The control of chronic low back pain has proven to difficult health problems in old ages. The goal of this study is to establish the benefit of nerve block for lumbar spinal pain in extremely elderly patients over 75 years of age.
Methods From March 2007 to August 2009, 46 consecutive patients with spinal pain (low back pain, radiating pain and claudication) performed medial branch block and dorsal root ganglion block. These patients were analyzed according to the demographic finding (age, sex, past medical history and past operation history), diagnosis, initial visual analogue scale score, treatment outcome and complications. Follow-up data were obtained by visual analogue scale score (VAS score).
Results Mean age was 79.8 (75-89 yrs) years old, and the male to female sex ratio was 1:2 (M:F=15:31). Chief complain was low back pain (76%), radiating pain (26%) and claudication (28.2%). They was diagnosed spinal stenosis (26.0%), facet syndrome (13%), herniated lumbar disc (17.4%), thoracic compression fracture (13%), Lumbar compression fracture (13%) and spondylolisthesis (17.4%). Averages of 2.5 nerve block were done. Initial mean VAS score was 7.29. All patients tolerated procedures well and were no significant complication with procedures. The mean VAS score decreased from 7.4 before nerve block to 4.2 after nerve block.
Conclusion The nerve block is considered as safe and effective therapeutic option for extremely elderly patients over 75 years of age who unsatisfactory to conservative managements including medication or physical therapy. |
