The Comparison of Bacterial Sputum Culture with Open versus Closed Endotracheal Suctioning System in Neurosurgically Intensive Cared Elderly Patients

Abstract
Objective
The aim of this study was to compare the effect of closed versus open endotracheal suctioning systems on the results of sputum bacterial culture in elderly patient.
Methods
In the period of 2010, older than 60 years patients were evaluated in this study. Patients were randomized to receive endotracheal suction with either closed suctioning system (CSS) or open suctioning system (OSS). Sputum culture was perfor- med at first 24 to 72 hours, and repeated sputum culture was performed 1 week later. Sputum culture result was reported semiquanti- tative method. Controlled group was defined as bacterial count did not change at the follow up culture. Uncontrolled group was defined as bacterial count increased.
Results
A total 59 patients was received endotracheal suction with open and closed suctioning system. Total 39 patients received open endotracheal suction and 20 patients received closed endotracheal suction. 17 patients were controlled group and 23 patients were uncontrolled group. In the controlled group, 7 patients (25.9%) received open endotracheal suction and 10 patients (76.9%) received closed endotracheal suction. The difference between closed suction system and open suction system had a static significance.
Conclusion
Following the change pattern of sputum culture result, closed suctioning system has better effectiveness on control the bacterial colonization than open suctioning system. Intensive care for elderly patients with closed suctioning system should be considered.
