The Possible Link Between Bone Mineral Density and Brain Atrophy and Hydrocephalus in Elderly

Abstract |
Objective Cerebral atrophy and hydrocephalus induced by the normal aging process, are known to lead dementia, memory disorder, and cognitive impairment. We hypothesized that cancellous bone consisted of collagen type 1 may be associated with cerebral atrophy and hydrocephalus. The purpose of the study was to evaluate the association between bone mineral density and cerebral atrophy and hydrocephalus.
Methods The scatterplot with a regression line was performed to graphically represent the association between T-score and Sylvian fissure ratio(SFR), Evans ratio, and bicaudate index(BCI). The area under the Receiver Operating Charac- teristic curve was used to determine the T-score for predicting the higher degree of brain atrophy and hydrocephalus. CT images that met BCI >0.2 and Evans >0.3 simultaneously were defined as hydrocephalus.
Results We included 458 patients having no stroke and traumatic brain injury, who underwent at least one or more dual-energy X-ray absorptiometry and brain CT simultaneously at our hospital from January 1, 2010, to December 31, 2016. The optimal cut-off values of the T-score for prediction of more brain atrophy were -1.50 and -2.55(AUC, 0.587; 95% CI, 0.529-0.644; p=0.001) in male and female, respectively. In addition, the optimal cut-off values of the T-score for prediction of hydrocephalus were -1.85 and -2.95 in male and female, respectively.
Results We found a significant negative relationship between SFR and systemic BMD in elderly women. In addition, we observed a tendency of more hydrocephalus in elderly patients with lower BMD. Further studies are needed to confirm our findings. |
