Surgical Management of Concurrent Tumor and Degenerative Disc Diseases of the Lumbar Spine in Elderly Patients. Lessons Learned from Complications

Abstract
Two cases of concurrent spinal intradural tumors and degenerative lumbar disc diseases are presented. Both patients showed recently developed thigh or leg pain, that were superimposed on the pre-existing lower back pain. Because of the ignorance of important physical examination denoting L3-4 root involvement and overconfidence of electromyographic results, the first patient suffered reoperation. In the second patient, omission of past history of long-term herbal medication resulted in accumulation of postoperative epidural bleeds. In this case report, we highlight the easily overlooked traditional way of history taking and physical examination for patient who await operative intervention. One-stage operation is mandated to attain satisfactory result and to avoid unneccessary reoperation, if possible.
