Clinical Outcome of Surgical or Endovascular Treatment for Ruptured Aneurysm in Elderly Patients

Abstract
Objective
The average life expectancy is rapidly increasing. Consequently, the number of elderly patients with aneurysmal subarachnoid hemorrhage is increasing. However, outcomes of elderly patients with subarachnoid hemorrhage remain unclear. The purpose of this study was to evaluate the results of surgical or endovascular treatment for ruptured aneurysm in elderly patients.
Methods
We retrospectively investigated elderly patients (ages 65-89 yr) who underwent surgical or endovascular treatment for ruptured aneurysm during the 5-year period 2003 to 2007. We compared the clinical features of patients of two age groups (group 1: ages 65-74 yr, group 2: ages >75 yr).
Results
Of the total of 180 patients, there were 28 patients (16% of total patients) in the elderly group 1 and 9 patients (4%) in the elderly group 2. We analyzed preoperative Hunt-Hess SAH classification, preoperative computed tomographic findings (Fisher grade) and Glasgow outcome scale between two elderly groups. Preoperative Hunt-Hess grade and computed tomographic finding were found to be significant predictors of favorable outcomes in both group 1 and 2. However, the age of the patient was not a predictor of outcome.
Conclusion
In the present study, we analyzed elderly patients who underwent surgical or endovascular treatment of ruptured aneurysms and compared patients in the elderly two groups. Preoperative clinical grade and CT findings influenced outcomes of both two groups. However, patient age was not an important predictor of outcome. The present study suggests that aggressive management for elderly patients who are good preoperative grade is important.
